- Basic: Exterior door has a gap at the threshold that opens to the outside. Observed : daylight shows through the back door when it is closed, at the top.
- Chemicals and toxic substances
- High priority: Tracking powder pesticide used inside establishment. Observed : white powder along the baseboards in the kitchen. The manager remembers pest control saying it was boric acid.

- Basic: Stored food not covered. Observed : in the kitchen reach-in cooler, cooked chicken, 41°, not yet covered. The manager tagged and cornered the pan.

- Basic: Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed : behind the front line soda machine, cups and debris.
- Insects, rodents, pests and animals
- High priority: Roach activity present as evidenced by live roaches found. Observed : 1 live roach going under water catch pan, beside the kitchen reach-in cooler. The manager removed with a paper towel.
- Basic: Exterior door has a gap at the threshold that opens to the outside. Observed : a gap at the top of the door when the door is fully closed. (Back door)
- Employee personal belongings
- Basic: Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed : a purse stored over bagged bread, on the back shelves. The purse was moved to the tool counter by the back door.

The inspector came back, found the problems fixed and allowed the business to reopen.
Conditions were a danger to public health, so the business was ordered to close immediately. It can't reopen until an inspector confirms the problems are fixed.
- Food-contact surfaces clean and sanitized
- Intermediate: Food-contact surface soiled with food debris, mold-like substance or slime. Observed black mold like substance on the back side of the ice maker shroud where the ice scoop sits.
- Handwash sinks and supplies
- Intermediate: No paper towels or mechanical hand drying device provided at handwash sink. Observed : lack of paper towels by the kitchen hand wash sink.
- Insects, rodents, pests and animals
- Basic: Dead roaches on premises. Observed : 1 dead roach next to the soda line chase. 1 dead under the bulk soda rack.These were swept up.
- High priority: Roach activity present as evidenced by live roaches found. Observed : 2 live roaches beside the soda chase. 2 live roaches came from behind the chase.
- Basic: Exterior door has a gap at the threshold that opens to the outside. Observed sunlight emits on three sides of the fully closed back door.

- Basic: Worn, torn and/or soiled floors. Observed : against the wall behind the cleaning supply rack, moist debris behind the leg of the rack.

- Basic: Wood food-contact surface not properly sealed. Observed : the wood below the water heating device is not sealed.
- Non-food contact surfaces clean
- Basic: Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed : dried ketchup spill below the bulk ketchup box on the front line. This was cleaned by staff. Corrected On-Site* ALSO, grease on the outside of the left fryer and around the table legs of the adjoining prep table.

- Basic: Exterior door has a gap at the threshold that opens to the outside. Observed : the back door when fully closed has light emitting at the top and the bottom.

- High priority: Toxic substance/chemical improperly stored. Observed : the first aid kits with ointment stored over the bulk soda. The manager moved them below.

- Basic: Carbon dioxide/helium tanks not adequately secured. Observed : multiple co2 tanks not secured in the kitchen. The chain was added.
