The inspector came back, found the problems fixed and allowed the business to reopen.

Conditions were a danger to public health, so the business was ordered to close immediately. It can't reopen until an inspector confirms the problems are fixed.

• Handwashing and hygienic practices
  • High priority: Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee using personal phone put on gloves without washing their hands first, left the kitchen area, returned and engaged in food preparation.
• Food-contact surfaces designed and maintained
  • Basic: Cutting board has cut marks and is no longer cleanable. Cutting board used in prep table with multiple deep cut marks.
  • Basic: Ice buildup in reach-in freezer and/or walk-in freezer. Frozen standing water in upright freezer across from walk in coolers.
• Insects, rodents, pests and animals
  • Intermediate: Nonservice animals in the food establishment or on premises. Cat sleeping on a seat in the dining area. Operator removed cat at the time of inspection.
  • Basic: Dead roaches on premises. 1 dead roach under tray jacks in dining room area.
  • High priority: Rodent activity present as evidenced by rodent droppings found. Approximately 7 rodent droppings on the floor next to beer upright cooler closer to the wall calendar, 2 droppings on the floor behind the soda syrups, in the bar area; approximately 3 droppings on the floor of heater room in dining area.
• Floors, walls, ceilings and ventilation
  • Basic: Ceiling, floors and walls soiled with accumulated food debris, grease, dust, or mold-like substance. Liquor and food walk in coolers.
  • Basic: Ceiling tile missing in women's restroom

Met the state's inspection standards; the inspector didn't need to come back. Any violations cited still count toward the grade.

• Non-food contact surfaces clean
  • Basic: Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple fans in kitchen with the presence of dust and food debris buildup.
• Floors, walls, ceilings and ventilation
  • Basic: Ceiling, floors and walls soiled with accumulated food debris, grease, dust, or mold-like substance. Liquor and food walk in coolers.
  • Basic: Ceiling tile missing in women's restroom.
• Chemicals and toxic substances
  • Intermediate: Spray bottle containing toxic substance not labeled. Sanitizer in spray bottle located at the dry storage, not labeled.
