    • Basic: Interior of oven has accumulation of black substance.

    • High priority: Employee failed to wash hands before putting on gloves to initiate a task working with food. Cook touched raw chicken then handled clean utensils and ready to eat food without first washing hands. Reviewed proper procedures and employee washed hands.

    • High priority: Live, small flying insects found. Approximately 2 small flying insects on wall above triple sink. Approximately 3 small flying insects on wall in dry storage room. Did not observe any flying insects land on food or food contact surfaces.
    • Basic: Dead roaches on premises. 1 dead under triple sink. 1 dead under rolling cart in front of triple sink. Operator removed and cleaned the areas.

    • Basic: Floor area(s) covered with standing water. Several areas of establishment with standing water - Cookline, dish area, back prep room, expo area.
