- Intermediate: Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed ceviche added to menu and not marked with consumer advisory. Observed oysters served under consumer advisory not marked as well. Educated operator on proper menu marking. Operator working on marking menu's.

- High priority: Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw eggs over sauce in Walk-in cooler and raw shrimp over pita bread in Reach-in cooler on cook line. Operator moved for proper storage.
- In-use utensils properly stored
- Basic: In-use utensil stored in standing water less than 135°F. Observed utensils stored in water reaching 100°F. Operator removed utensils from water and placed for cleaning.
- Food-contact surfaces clean and sanitized
- High priority: Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed dish machine testing at 0ppm. Operator called technician and set up manual sanitizer. Operator fixed dish machine before end of inspection, now testing at 100ppm.

- High priority: Toxic substance/chemical improperly stored. Observed sanitizer stored above prep table. Operator moved for proper storage.

Conditions were a danger to public health, so the business was ordered to close immediately. It can't reopen until an inspector confirms the problems are fixed.

- Basic: Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed 13 tuna filets bearing label to remove from packaging before thawing thawed in reach in cooler.

- High priority: Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw scallops over milk in Walk-in cooler. Repeat from 5/19/2026.

- High priority: Nonfood-grade containers used for food storage - direct contact with food. Observed flour in non food grade dish machine bucket and bread crumbs in non food grade storage containers.

- High priority: Rodent activity present as evidenced by rodent droppings found. Observed 40 rodent droppings in old dish pit area, 10 rodent droppings on shelves containing parchment paper and styrofoam cups, 6 rodent droppings on soda boxes, 4 in dry storage in coffee filter bag.
