Conditions were a danger to public health, so the business was ordered to close immediately. It can't reopen until an inspector confirms the problems are fixed.
- Approved source; food safe and unadulterated
- High priority: Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed ready house made salads (cut lettuce) hard boiled eggs, sliced cheddar cheese (47°F-51°F - Cold Holding) in reach-in cooler at front service line. Foods were in cooler overnight. See Stop Sale.
- Food out of temperature
- High priority: Time/temperature control for safety food cold held at greater than 41°F. Observed ready house made salads (cut lettuce) hard boiled eggs, sliced cheddar cheese (47°F-51°F - Cold Holding) in reach-in cooler at front service line. Foods were in cooler overnight. See Stop Sale.
- Food protection and cross-contamination
- Basic: Food stored on floor. Observed two boxes with sweet potatoes on floor in walk-in-cooler.
- Handwashing and hygienic practices
- High priority: Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee remove ear bud from ear while prepping dressings and then put back in ear and continued to prep dressings without washing hands in between and put on gloves. Discussed with operator.
- Basic: Employee eating in a food preparation or other restricted area. Observed employee eating at front service line while employee prepping dressings.
- Food-contact surfaces designed and maintained
- Basic: Equipment in poor repair. Observed gaskets torn on reach in freezer in prep area, and gasket on reach-in cooler salad cooler at front line torn.
- Basic: Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed multiple shelving in walk-in-coolers with rust that has pitted surface.
- Basic: Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed reach-in cooler at front line used for salads ambient temperature 50°F. Observed ice build up on fan in back of cooler. Also, walk-in-cooler with meat ambient 58°F. Operator stated it was in defrost. Operator called to have serviced. Retemped 15 min later at 55°F.

- Basic: Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior of large ice machine soiled with mold like substance.
- Non-food contact surfaces clean
- Basic: Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gasket on walk-in-cooler soiled with debris.
- Clean equipment storage and air drying
- Basic: Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed two ice buckets at large ice machine not protected/ inverted. Operator inverted both ice buckets.
- Insects, rodents, pests and animals
- High priority: Live, small flying insects found. Observed one flying insect on floor in beverage station.
- Basic: Dead roaches on premises. Observed approximately 27 dead roaches in the following areas: 7 under food warming boxes near AC closet 2 on floor under hand wash sink in smoker room 3 on floor under rolling bread racks 1 on shelf below front counter 1 on lid of box of to go bags at front line area 6 on floor in front service line area 2 on floor next to deep freezer near prep area 1 in tray of warm towel box 1 on top of dishwashing machine 3 on floor under dishwashing machine

Observed 3 live roaches in the following areas: 2 on shelf below front counter 1 on floor between beverage station and food warmer table. Employee stepped on live roaches observed between beverage station and food warmer table.
- High priority: Roach excrement and/or droppings present. Observed 3 roach sacks in the following areas: 2 on floor under electrical panel 1 under hand wash sink in room with smoker.
