- High priority: Live, small flying insects found. Observed 6 flying insects in establishment at time of callback. Person in charge killed and discarded all but one flying insect during inspection.

Conditions were a danger to public health, so the business was ordered to close immediately. It can't reopen until an inspector confirms the problems are fixed.

- High priority: Stop Sale issued due to food not being in a wholesome, sound condition. Observed ingredients at pizza make unlidded, flying insect observed in contact with minced garlic in pan.
- Food out of temperature
- High priority: Time/temperature control for safety food cold held at greater than 41°F. Canadian bacon on pizza make stored above fill line at 54°F. Person in charge removed excess and placed in unit drawers below.

- Basic: Food stored in a location that is exposed to splash/dust. Observed unlidded container of dough stored below and to the right of the hand wash sink exposed to possible splash from hand wash sink. Person in charge covered and moved below dough press

- High priority: Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker missing at mop sink on non chemical side of splitter with black hose attached. Also, observed vacuum breaker on mop sink faucet corroded, missing pieces.

Observed ingredients unlidded at pizza make, flying insect observed in pan of minced garlic. See stop sale. Observed approximately 10 flying insects in back kitchen area near dishpit and mop sink area. Person in charge began killing.

- Basic: Floor soiled/has accumulation of debris. Floors at front pizza make area with debris accumulated in corners along walls, at base of cabinet at point of sale system with presence of flying insects in area. Also, floor soiled in back kitchen area at dishmachine and triple sink area.
