    • Basic: Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Hood filters have grease build up. Gaskets on front line cold bar doors are soiled with food debris.

    • Basic: Bathroom facility not clean. Toilet in mens restroom is dirty.
  • Insects, rodents, pests and animals
    • High priority: Live, small flying insects found. Approximately 132 live flying insects observed in the following areas; Approximately 100 under the counters at the main make line, concentrated under the steam wells.   • Floors, walls, ceilings and ventilation
    • Basic: Floor/wall/ceiling/vent soiled/has accumulation of debris. Ceiling tiles around the vents on the make line have dust. Floors under steam wells and fryer with food debris. Walls in mop sink area are soiled.

    • Basic: Current Hotel and Restaurant license not displayed. Displayed license expired June 1,2026. Operator provided copy of current license during the inspection.

    • Intermediate: Food service manager not certified within 30 days of employment. Manager on duty at the time of inspection hired 3-4 months ago with no manager food handler training.
