At callback observed approximately 8 dead roaches in the following areas: 2 in cabinet at front service line, 1 below bag in box storage shelf, 1 on side of reach-in cooler at coffee brewing station, 1 at mop sink, 2 under rolling cart below turbo oven, and 1 at 3CS. Operator had employee clean the dead roaches up and discard.
    • High priority: Roach activity present as evidenced by live roaches found. At callback observed approximately 4 live roaches I the following areas: 1 in cabinet at front service station, 2 under rolling cart with food warmer, 1 on floor at front counter. Also, observed two roach sacks on side of reach-in cooler at front service station. Operator and employee killed and discarded live roaches and roach egg sacks.

Conditions were a danger to public health, so the business was ordered to close immediately. It can't reopen until an inspector confirms the problems are fixed.

  • Food out of temperature
    • High priority: Time/temperature control for safety food cold held at greater than 41°F. Observed half and half (55°F - Cold Holding) in front service cooler at coffee equipment. Also, raw pork (47°F - Cold Holding) in standup glass door reach-in cooler in kitchen. Also, sliced American cheese, butter (67°F-69°F - Cold Holding) on top of lid of glass door reach-in cooler in prep area. Operator stated he had items out while preparing orders. Operator placed both items in the coolers to chill.
  • Food-contact surfaces designed and maintained
    • Basic: Equipment in poor repair. Observed handle on microwave in disrepair/ bottom part of handle not attached.
  • Food-contact surfaces clean and sanitized
    • Intermediate: Food-contact surface soiled with food debris, mold-like substance or slime. Observed deflector plate in large ice machine and ice chute on self dispensing soda machine soiled with residue. Also, slicer blade and surrounding area soiled with accumulation of dried up food. Also, cutting board at glass door reach-in cooler soiled/ stained.
    • Basic: Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed interior of turbo over has an accumulation of food debris.
    • Basic: Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior of large ice machine bin soiled with black like substance.

    • Basic: Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed exterior of trash can across from glass door reach-in cooler soiled.
  • Clean equipment storage and air drying
    • Basic: Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed large ice bucket not stored inverted across from large ice machine.
  • Single-service items
    • Basic: Single-service articles improperly stored. Observed box with wrapped sleeves of single use cups in back storage room stored on floor.

    • High priority: Live, small flying insects found. Observed one flying insect next to self dispensing soda machine and one behind door leading to dry food storage area.
    • Basic: Dead roaches on premises. Observed approximately 22 dead roaches in the following areas: 6 under trip,e sink, 3 under prep table, 3 under rolling cart by turbo oven, 4 next to large ice machine, 5 on floor in back storage area, 3 under rolling cart across from front counter. Operator called maintenance to have cleaned up.
    • High priority: Roach activity present as evidenced by live roaches found. Observed approximately 4 live roaches in the following areas: 1 on wall next to bread shelf, 2 on floor in kitchen, and 1 on floor behind door at dry food storage area. Operator killed live roaches. Also, and 4 roach sacks in back dry food storage room.

    • Basic: Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items.observed employee personal belongings on shelf next to coffee filters on dry food storage shelf.

    • High priority: Toxic substance/chemical improperly stored. Observed spray bottle containing toxic substance stored next to fruit syrups on bottom shelf of prep table.
    • Intermediate: Spray bottle containing toxic substance not labeled. Observed unlabeled Spray bottle containing toxic substance stored on bottom shelf at prep table. Operator stated he will discard spray bottle of cleaner.

    • Basic: Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in ringer of mop bucket in a manner to not allow mop to air dry.
