- High priority: Roach activity present as evidenced by live roaches found. One live roach crawling on shelf inside kitchen at front counter.
- High priority: Roach excrement and/or droppings present. One roach egg sac on floor behind equipment at front counter.
- Basic: Outer openings not protected during operation and vermin and/or environmental cross contamination present. Duct tape used to cover holes at front counter. Manager stated they will find a smooth and cleanable solution.

Conditions were a danger to public health, so the business was ordered to close immediately. It can't reopen until an inspector confirms the problems are fixed.
- Food out of temperature
- High priority: Time/temperature control for safety food cold held at greater than 41°F. Inside reach-in cooler at bottom of ice cream machine: ice cream mix (46°F - Cold Holding). The manager stated the ice cream mix has been stored inside the cooler overnight and it had not been removed today. The ambient air temperature inside the cooler is 49°F. Stop sale issued.

- Basic: Food-contact surface not smooth and easily cleanable. Interior of reach-in cooler repaired with duct tape.
- Basic: Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. The ambient air temperature inside the cooler in bottom of ice cream machine is 49°F. Stop sale issued on ice cream inside unit.
