    • Basic: Equipment in poor repair. At time of callback inspection observed broken reach in cooler next to fryers, not in use. Observed pizza prep reach in cooler lid broken.
    • Basic: Walk-in cooler/freezer shelves with rust that has pitted the surface. At time of callback inspection observed walk in cooler shelves rusted.
  • Plumbing and backflow prevention
    • Basic: Plumbing system in disrepair. At time of callback inspection observed hand sink in kitchen in disrepair, hanging off of wall.
  • Handwash sinks and supplies
    • Basic: No handwashing sign provided at a hand sink used by food employees. At time of callback inspection observed no hand washing sign at both hand sinks in kitchen area.
  • Floors, walls, ceilings and ventilation
    • Basic: Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. At time of callback inspection observed ceiling heavily dusty above bar area.

Conditions were a danger to public health, so the business was ordered to close immediately. It can't reopen until an inspector confirms the problems are fixed.

  • Food out of temperature
    • High priority: Time/temperature control for safety food cold held at greater than 41°F. Observed shredded cheese (56°F - Cold Holding); sausage (55°F - Cold Holding); shredded mozzarella (55°F - Cold Holding); pizza sauce (54°F - Cold Holding) inside reach in cooler at pizza prep area, as per operator for 2 hours. Observed shredded cheese (45°F - Cold Holding); shredded lettuce (46°F - Cold Holding); raw chicken (45°F - Cold Holding) inside reach in cooler across from walk in cooler.
  • Food protection and cross-contamination
    • High priority: Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw bacon stored over soft pretzels inside walk in cooler. Operator relocated products.
  • In-use utensils properly stored
    • Basic: Ice bucket/shovel stored on floor between uses. Observed ice buckets stored on floor at bar area.
  • Handwashing and hygienic practices
    • High priority: Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee handle raw chicken then change gloves and begin to handle clean equipment without washing hands.
    • High priority: Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee grab trash can then handle pizza paddle to plate pizza without changing gloves and washing hands.

    • Basic: Dead roaches on premises. Observed one dead roach on stairs leading up to dry storage room.
    • High priority: Rodent activity present as evidenced by rodent droppings found. Observed approximately 20+ rodent droppings on deli slicer underneath prep table in pizza prep area and approximately 15+ droppings on shelf where pizza boxes are stored. Operator removed deli slicer and began cleaning up droppings.

    • Basic: Floor area(s) covered with standing water. Observed standing water on floor in server room underneath soda machine.
    • Basic: Wall soiled with accumulated grease, food debris, and/or dust. Observed wall soiled in dish area by dish machine and in server room.
    • Basic: Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling heavily dusty above bar area.
    • Basic: Floor soiled/has accumulation of debris. Observed floor heavily soiled in pizza prep area and floor in server station soiled with slime like substance.
